What is the Initial Contact Form?
The Initial Contact Form is a structured questionnaire designed to help you describe your current experiences and any difficulties you may be facing.
It is intended as an accessible starting point for people who are unsure what to do next and want to better understand their situation before making any decisions.
For some people, this provides a helpful way to organise their thoughts and reflect on their experiences in a structured format, particularly if they are finding it difficult to explain things clearly or know where to begin.
This is therefore something that you can use to support in speaking with GP about a referral to an NHS funded assessment.
What happens when I complete the form?
Once completed, your responses are reviewed and used to generate an Initial Contact Report.
This report is sent to you by email and provides an overview of the areas identified within your responses. It is designed to support understanding and help you consider whether further support or assessment may be helpful. The information is also used in our assessment process if you access our service.
There is no automatic progression from completing the form to any further service. It is simply a tool to support understanding and decision-making, although you are given an opportunity to book with us should you wish to.
We store this information for up to 28 days and only keep it if you go on to work with us.
Is this the same as a diagnosis?
No — the Initial Contact Form and report do not provide a diagnosis.
They are designed to support understanding and help identify whether further assessment or support may be helpful. The advice usually extends way beyond whether or not an assessment is advisable.
Many people use it as a starting point before deciding what to do next, rather than as a standalone outcome.
Will divergence contact me after I complete the form?
No — there is no automatic follow-up.
You will only be contacted if you have explicitly requested this as part of the form.
This is intended to ensure that you remain in control of what happens next.
What happens to the information I provide?
The information you provide is used to generate your report and support your enquiry.
If you choose to continue with divergence, it may form part of your clinical record.
If you do not take any further action, the information is not retained long term and will be deleted in line with data protection policies.
How can this be helpful to me?
The Initial Contact Form can be helpful if you are unsure where to start.
It can:
· Provide a structured way to think about your experiences
· Help you understand what might be going on
· Highlight areas that may need further understanding
· Support conversations with professionals such as your GP
· Help you decide whether assessment or support may be appropriate
For some people, this clarity alone is valuable, even if they do not go on to pursue further assessment.
Who is this most useful for?
This may be useful for people who:
· Are unsure whether they may have ADHD / Autism or related difficulties
· Want to better understand their experiences
· Are considering whether to seek assessment
· Want something structured to take to a GP or other service
It can also be useful for people who find it difficult to explain their experiences verbally and benefit from having something written to refer to.
Can I complete the form more than once?
Yes — if your situation changes or you wish to reflect on your responses at a different time, you may complete the form again.
This can sometimes provide additional insight, particularly if your understanding of your experiences develops over time.
How do you protect my privacy?
Our clinic is regulated by the Care Quality Commission who have clear standards that any health and social care provider must meet. We use "Semble" as our records system and they are continually working to ensure that the highest standards of data security are maintained.
